Pharmacological properties
Sulfadimezine is a short-acting sulfonamide drug. It is active against gram-positive and gram-negative cocci, Escherichia coli, Shigella, Klebsiella, Vibrio cholerae, pathogens of gas gangrene, anthrax, diphtheria, catarrhal pneumonia, plague, as well as Chlamydia, Actinomyces, and Toxoplasma gondii. It has a bacteriostatic effect. The mechanism of action is related to para-aminobenzoic acid (PABA) and the competitive inhibition of dihydropteroate synthase, which leads to disruption of tetrahydrofolic acid synthesis necessary for purine and pyrimidine synthesis.
Method of administration and dosage
Sulfadimezine should be taken orally.
The average dose for adults is 2 g (4 tablets) for the first dose, then 1 g (2 tablets) 4–6 times a day.
Maximum adult doses: single – 2 g, daily – 7 g.
For children aged from 3 years, prescribe 0.1 g/kg of body weight for the first dose, then 0.025 g/kg of body weight every 4–6–8 hours.
For treatment of shigellosis, the regimen for adults is:
Days 1–2: 1 g 6 times a day (every 4 hours);
Days 3–4: 1 g 4 times a day (every 6 hours);
Days 5–6: 1 g 3 times a day (every 8 hours).
Total for the treatment course – 25–30 g of the drug.
After a 5–6-day break, a second course should be administered:
Days 1–2: 1 g 5 times a day (every 4 hours, with 8-hour intervals at night);
Days 3–4: 1 g 4 times a day (not at night);
Day 5: 1 g 3 times a day.
During the second course, 21 g of Sulfadimezine is taken. In mild cases of shigellosis, the dose may be reduced to 18 g.
Dosage for treating shigellosis in children aged from 3 years: single dose – from 0.4 g to 0.75 g, to be taken 4 times a day. The drug should be taken for 5–7 days.
For most infectious diseases, treatment should continue for at least another 48–72 hours after the disappearance of symptoms and confirmation of recovery by bacteriological analysis.
